Chip Tynan responded to my query about aggressive Mute Swans with the 
message below.

In addition to this, my google search turned up many instances of Mute 
Swans attacking water craft (canoes, kayaks and even jet skis).

My thanks to all who responded.  So far, this is the only reference to 
swan-caused human deaths:

your memory is good as gold.  It appears that on two occasions mute 
swans not only killed an "Indian fisherman" in 1982, but a Massachusetts 
child "around 1930" - both way back before the desktop computer age.
